    Komposisi:

    - Midazolam 1 mg/mL.

    Bentuk sediaan:

    - Cairan injeksi dalam ampul 5 mg/5 mL.

    Farmakologi:

    Merupakan penghambat susunan saraf pusat golongan benzodiazepine yang memiliki efek sedasi, induksi tidur yang cepat, anti konvulsan, dan relaksasi otot, serta tidak memperlambat waktu pulih sadar dari anestesia umum pada pasien dewasa. Bioavailabilitas IM >90%. Onset sedasi 15 menit dan puncak sedasi 30-60 menit setelah injeksi IM. Onset sedasi 3-5 menit setelah injeksi IV. Ikatan protein plasma lebih kurang 94%. Dimetabolisme sempurna dan dieliminasi melalui ginjal.

    Indikasi:

    Premedikasi, induksi anestesi umum, sedasi.

    Dosis:

    - Premedikasi:

    Dewasa: 70-100 mcg/kg IM. Dosis lazim: 5 mg.

    Orang tua & pasien lemah: 25-50 mcg/kg IM.

    Diberikan 30 menit sebelum operasi.

    - Induksi:

    200-300 mcg/kg IV lambat. Anak > 7 th: 150 mcg/kg IV lambat.

    - Sedasi:

    2 mg IV, dapat ditingkatkan 0,5-1 mg. Dosis rata-rata 2,5-7,5 mg.

    - Sedasi di ICU:

    30-300 mcg/kg infus IV selama 5 menit, kemudian 30-200 mcg/kg/jam.

    Kontraindikasi:

    Hipersensitif terhadap benzodiazepine, glaukoma akut sudut sempit, bayi prematur.

    Peringatan dan Perhatian:

    - Hati-hati pada pasien dengan miastenia gravis, insomnia pada psikosis, depresi berat, kerusakan otak berat, insufisiensi pernapasan, usia lanjut, ibu hamil, gangguan hati & ginjal.

    - Dapat menyebabkan ketergantungan.

    - Hindari penghentian tiba-tiba.

    - Dapat mengganggu kemampuan mengendarai dan menjalankan mesin.

    Efek Samping:

    Mual, muntah, sakit kepala, cegukan, laringospasme, sesak napas, halusinasi, ataksia, ruam kulit, reaksi paradoksikal, episode amnesia.

    Composition:

    Presentation:

    - Injection liquid in ampoules 5 mg/5 mL. - Injection liquid in ampoules of 15 mg/3 mL.

    Pharmacology:

    It is a inhibitor of the benzodiazepine central nervous system which has the effect of sedation, rapid sleep induction, anti-convulsants, and muscle relaxation, and does not slow down the time to recover consciously from general anesthesia in adult patients. IM bioavailability> 90%. Onset of sedation of 15 minutes and peak sedation 30-60 minutes after IM injection. Onset of sedation 3-5 minutes after IV injection. Plasma protein binding is approximately 94%. Perfectly metabolized and eliminated through the kidneys.

    Indications:

    Premedication, induction of general anesthesia, sedation.

    Contraindications:

    Hypersensitivity to benzodiazepines, acute narrow-angle glaucoma, premature infants

    Dosage:

    - Premedication:   Adults: 70-100 mcg / kg IM. Common dosage: 5 mg.   Geriatrics & weak patients: 25-50 mcg / kg IM.   Given 30 minutes before surgery. - Induction: 200-300 mcg / kg IV slow. Children >7 years: 150 mcg / kg IV slow. - Sedation: 2 mg IV, can be increased from 0.5 to 1 mg. The average dose is 2.5-7.5 mg. - Sedation in the ICU: 30-300 mcg / kg IV infusion for 5 minutes, then 30-200 mcg / kg / hour.

    Warning and Precautions:

    - Beware of patients with myasthenia gravis, insomnia in psychosis, major depression, severe brain damage, respiratory insufficiency, advanced age, pregnant women, liver & kidney disorders. - Can cause dependence. - Avoid sudden stopping. - Can interfere with the ability to drive and run the engine.

    Adverse events:

    Nausea, vomiting, headache, hiccups, laryngospasm, shortness of breath, hallucinations, ataxia, skin rash, paradoxical reactions, episodes of amnesia.

    Drugs Interaction:

    Fortanest enhances the central sedative effects of neuroleptics, tranquilizers, antidepressants, sleep-inducing drugs, analgesics, anaesthetics. In cases certain this potentiation provides therapeutic benefits. Special attention must be given against the possibility of potentiation in patients with certain risks. On in a number of cases, the mutual potential of alcohol and Fortanest results unexpected reaction (drinking alcohol is not allowed for at least 12 hours after parenteral administration). Potentiation arises in shared use erythromycin, ketoconazole, itraconazole, diltiazem, verapamil, cimetidine, and inhibitors hepatic enzyme.
