    Komposisi:

    Domperidone 10 mg dan 5 mg/5mL

    Bentuk sediaan:

    Tablet dan sirup.

    Farmakologi:

    Efek anti emetik dapat disebabkan oleh kombinasi efek periferal (gastrokinetik) dengan antagonis terhadap reseptor dopamine di kemoreseptor “ trigger zone” yang terletak di luar sawar darah otak di area postrema.

    Indikasi:

    Untuk mual dan muntah akut, mual dan muntah yang disebabkan oleh pemberian levodopa dan bromokriptin lebih dari 12 minggu, pengobatan gejala dispepsia fungsional.

    Anak-anak :untuk mual dan muntah pada kemoterapi kanker dan radioterapi.

    Dosis:

    Dispepsia fungsional :

    Dewasa : 10 - 20 mg 3 kali sehari dan 10 – 20 mg sekali sebelum tidur malam tergantung respons klinik. Pengobatan jangan melebihi 12 minggu

    Mual dan muntah (termasuk yang disebabkan oleh levodopa dan bromokriptin) :

    Dewasa (termasuk usia lanjut) : 10 – 20 mg sehari, interval waktu 4 - 8 jam.

    Anak-anak (sehubungan kemoterapi kanker dan radioterapi) : 0,2 - 0,4 mg/kg berat badan sehari, dengan interval waktu 4 - 8 jam.

    Obat diminum 15 - 30 menit sebelum makan dan sebelum tidur malam.

    Kontraindikasi:

    Penderita hipersensitif terhadap domperidone.

    Anak-anak (kecuali untuk mencegah mual dan muntah sehubungan kemoterapi kanker dan radioterapi).

    Penderita dengan prolaktinoma tumor hipofise yang mengeluarkan prolaktin.

    Peringatan dan Perhatian:

    Hati-hati penggunaan pada wanita hamil dan menyusui.

    Tidak dianjurkan penggunaan jangka panjang.

    Hati-hati penggunaan pada penderita gangguan fungsi hati dan ginjal.

    Efek Samping:

    Jarang dilaporkan : sedasi, reaksi ekstrapiramidal, distonik parkinson, tardive dyskinesia (pada pasien dewasa dan usia lanjut) dan dapat diatasi dengan obat antiparkinson.

    Peningkatan prolaktin serum sehingga menyebabkan galaktore dan ginekomastia. Mulut kering, sakit kepala, diare, rash kulit, rasa haus, cemas, gatal.

    Composition:

    Each tablet contains: Domperidone 10 mg Each 5 mL syrup contains: Domperidone 5 mg

    Presentation:

    DOMPERIDONE tablet: Box of 10 strips x 10 tablets DOMPERIDONE syrup: Box of 1 bottle 60 mL

    Pharmacology:

    • Domperidone is a dopamine antagonist that has antiemetic activity. • The antiemetic effect can be caused by a combination of peripheral effects (as gastrokinetic) with dopamine receptor antagonists on CTZ located outside the blood brain barrier in the postrema area. Domperidone does not penetrate the blood brain barrier so extrapyramidal effects are very rare. • Peroral administration of domperidone prolongs antro-duodenal contractions, increases gastric emptying in liquid and semi-solid form in healthy people, and forms solid in patients with delayed gastric emptying time, and ncreases pressure on the lower esophageal sphincter in healthy people.

    Indications:

    Adult: • Treatment of acute nausea and vomiting. Routine prevention of vomiting after surgery is not recommended. • Treatment of nausea and vomiting induced by levodopa and bromocriptine treatment. Domperidone should not be used more than 12 weeks. • Treatment of functional dyspepsia symtomps. Not recommended for long term administration. Children: • Not recommended, except for nausea and vomiting caused by cancer chemotherapy and radiotherapy.

    Contraindications:

    • Patients with known hypersensitivity to domperidone. • Patient with prolactinoma which produces prolactin. • Patient whenever stimulation of gastric motility might be dangerous eg, in the presence of gastrointestinal bleeding, perforation, or mechanical obstruction.

    Dosage:

    • Functional dyspepsia: Adults and elderly: 10-20 mg 3 times per day and 10-20 mg once before going to bed at the night depending on the clinical response. Treatment should not exceed 12 weeks. • Nausea and vomiting (including those caused by levodopa and bromocriptine): Adults (including elderly): 10-20 mg, interval: 4-8 hpurs. Children (related with cancer chemotherapy and radiotherapy): 0.2-0.4 mg/kg daily, interval: 4-8 hours. The drug should be taken 15-30 minutes before meals and before going to bed at the night.

    Warning and Precautions:

    • Caution should be taken in pregnant and nursing women. • Not recommended for long term treatment. • Caution should be taken in patients with liver and renal dysfunction.

    Adverse events:

    • Rarely: sedation, dystonic extrapyramidal reactions, parkinson, tardive dyskinesia (in adult and elderly patients) and can be treated with antiparkinson drugs. • Increased serum prolactin resulting in galactorrhea and gynecomastia. • Dry mouth, headache, diarrhea, skin rash, thirst, anxiety and itching.

    Drugs Interaction:

    • Domperidone decrease the effect of hypoprolactinemia of bromocriptine. • Concomitant administration with muscarinic anticholinergic and opioid analgesic will antagonize the effect of domperidone. • Concurrent use with antacid will decrease the bioavailability of domperidone. • Bioavailability of domperidone may increase from 13% to 23% if taken 1.5 hours after meal.
